Pricing: $50 per claim file, 15% processing fee per claim reimbursement
An application to request information needed for this package will be shared via email.
Our team will check if the patient's eligibility for coverage before moving forward with the service.
We will need the following:
- Patient Name
- Patient Member ID
- Patient DOB
- Insurance Company and State
